This survey is for you who lives in an older people’s care home. Please answer on the basis of your experience of the service during the past year. All of your answers will be treated as confidential.
Calderdale Council and NHS Calderdale Clinical Commissioning Group along with other providers, and professionals are committed to working together to build sustainable and quality services to improve the quality of life for all older people who live in nursing or residential care homes.
The results from this survey will inform our work to build a sustainable model of older people’s care home provision that enables people to live as independently as they can in a home that promotes choice, dignity, control and quality of life.
